U.S. Department of State Passport Consent Form(DS-3053)

The DS-3053 is used when a passport needs to be obtained for a minor child and only one parent is able to attend the appointment. What do I need to do to do?
1. Download Form DS-3053 from the official passport website.
2. Present valid government-issued ID
3. Sign form in front of a notary (Do NOT pre-sign this document)
4. Attach a copy of your ID
5. Submit with the passport application.
